Who you are:

-Photo ID (driver's license, state ID, or passport)

-Social Security numbers for everyone in your household applying

-Proof of citizenship or immigration status if applicable

Where you live:

-Proof of address (a utility bill, lease agreement, or piece of official mail with your name and address on it)

Your money and income:

-Pay stubs from the last 30 days for everyone in the household who works

-If self-employed, records of income and expenses

-Proof of any other income–Social Security, child support, unemployment, etc.

-Most recent bank statements

Your expenses:

Rent or mortgage amount

Utility bills (electric, gas, water)

Child care costs if applicable

Medical expenses if anyone in the household is elderly or disabled

Your household:

-Birth certificates for any children applying

-Proof of any dependent care costs

One practical note: you don't need to have every single one of these to walk in and start the process. Bring what you have. The caseworker will tell you what's missing and you can provide it afterward. Don't let an incomplete document pile stop you from going.
